Terms and conditions for Monthly Membership
MONTHLY MEMBERSHIP AGREEMENT
This contract made between the client and FIT4MOM Dunedin-Palm Harbor is a month to month agreement and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited classes.AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Dunedin-Palm Harbor to initiate a charge to the credit card number entered into your Pike 13 account for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM franchise owner.
I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Dunedin-Palm Harbor to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid for up too 4 months, it will be cancelled.
CANCELLATION POLICY
This Monthly membership plan requires 3 month commitment. If you choose to cancel prior to the 3 payment requirement, you will be responsible for the remaining required payments. After the 3 month commitment, a 30 day cancellation notice to Amanda Seaton. The 30 day period starts on your payment day. Cancellation after to the 30 day cancellation period will result in not refunding the current charge or next charge. The 30 days begins the day the written notice is received.
To cancel this membership, the member must contact the FIT4MOM franchise owner 30 days prior to their next payment due date to request a cancellation. No refunds will be given for dues already paid. This membership is not transferable to another person or anywhere outside the FIT4MOM Dunedin-Palm Harbor franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged the full registration fee.
HOLD POLICY
Memberships may be held up to a 4 months per hold period. Hold starts will coincide with the next billing date and can end on any day of the month. When a hold period is ended, the monthly auto payments will resume on the return date. There will be a $20 processing fee charged to your credit card on file for the hold period. Any memberships on hold for more that 4 months, will be charged a $20 hold fee per month.
INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement, written notice must be received by FIT4MOM Dunedin-Palm Harbor within the initial ten days. You are responsible for paying for the classes already attended.

With your paid monthly membership you are given free access to FIT4MOM On Demand. You will have unlimited access to On Demand workouts. When your FIT4MOM Dunedin-Palm Harbor membership properly canceled according to the terms and conditions, your On Demand subscription will automatically canceled. If you would like to keep your On Demand subscription, you can re-enroll using the same login information and purchase your desired paid subscription.
