Terms and conditions for Single Class Pass

FIT4MOM DUNEDIN-PALM HARBOR CLASS PASS AGREEMENT

This contract is made between the client and FIT4MOM Dunedin-Palm Harbor and is a 1 class pass. This pass does expire, regardless of attended classes, 1 month after the purchase date. This pass allows you to attend 1 class of choice.

INITIAL 10-DAY CANCELLATION POLICY

You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM Dunedin-Palm Harbor within the initial ten days. No refunds will be given for unattended visits. AUTHORIZATION FOR PAYMENT

I authorize FIT4MOM Dunedin-Palm Harbor to initiate a charge to the credit card number entered into Front Desk for my dues.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Dunedin-Palm Harbor to fulfill my payment commitment and I am obligated to pay. Dues will be drawn from the account on file on the day of purchase. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a class pass remains unpaid for 4 months, it will be cancelled.

AGREEMENT

By purchasing a class pass, you are agreeing to the terms and conditions of this agreement. The terms of this agreement may change with proper advance written notice.
